Affordable Health Coverage

The Primary Care & Hope Clinic has Certified Application Counselors on staff to assist with enrolling in affordable health insurance coverage or TennCare through the Health Insurance Marketplace/ Healthcare.gov.  For more information or to make an appointment, call 615-893-9390 and press 1.

Open Enrollment for 2018 health coverage has ended.

You can enroll if you have life changes like losing health coverage or having a baby, or if you qualify for TennCare or CoverKids


To apply for coverage in the Health Insurance Marketplace, you will need to provide information about you and your household, including income, any insurance you currently have, and some additional items.  Below is a checklist of items to bring if you come in for assistance with applying:

  • Social Security Numbers (or document numbers for legal immigrants)Need health coverage? The Health Insurance Marketplace is open! Apply Now
  • Employer and income information for every member of your household who needs coverage (pay stubs or W-2 forms)
  • Policy numbers for any current health insurance plans covering members of your household
  • A completed Employer Coverage Tool for every job-based health insurance plan you or someone in your household is eligible for. (You will need to fill out this form even for coverage you are eligible for but do not enroll in.

Health Insurance Basics:

  • Premium: The money you pay the insurance company to buy the plan.
  • Deductible: The amount you have to pay for your health care each year before your insurance starts paying for care.
  • Copay/Co-insurance: The money that you have to pay “out of pocket” for each service you receive.
  • Out-of-pocket limit:  The most you will have to pay each year for care covered by your plan.
  •  Provider Network: The doctors and other health care providers who have agreed to work with your health insurance company.
