PATIENT SERVICES REPRESENTATIVE (PSR)
Looking for a friendly, customer-service oriented individual to provide excellent service to our patients and further the mission of the Primary Care & Hope Clinic.
Essential Functions
- Represent PC&HC to the patients in a courteous and professional manner. Provide prompt, efficient and accurate patient service.
- Greet, assist and direct patients appropriately.
- Assist patients in making appointments with assigned provider.
- Assist in Call Center by answering telephone, screens calls, provides information to patients and other callers as directed.
- Maintain a strong customer focus while working collaboratively within the teams multiple demands and patient needs.

- Treat all patient information confidentially and in accordance with established HIPAA regulations.
- Make patient appointments in accordance with PC&HC scheduling guidelines.
- Register patients, obtain and enter all necessary demographic and insurance related information needed as record for registration and billing of all patients’ visits.
- Coordinate information from patients to ensure accurate billing and maximum reimbursement.
- Establish a new electronic patient account/record and accurately enters all patient information into the computer system according to established procedures.
- Update and correct patient account information in the computer system during each visit.
- Ensure new and updated documentation is filed in the medical record.
- Medical Record administration.
- Verify insurance coverage eligibility prior to the provision of services according to established procedures.
- Inform patient of and collect co-pays and deductibles prior to scheduled services.
- Prepare and issues receipts, process credit card payments.
- Provide information on account status.
- Improve the patient experience and health care processes and help assure the provision of high quality health care, as well as an ongoing process of performance improvement.
- Maintain a work area that reflects a professional environment at all times.
Marginal Functions
- Assist in the opening and closing security procedures of the building.
- Fill-in for absenteeism, including at other PC&HC locations.
- Perform all other duties as required or assigned.
Requirements:
- Must be 18 years of age or older.
- Must have a High School diploma or equivalent.
- Must have ability to manage a fast paced environment without sacrificing efficiency and accuracy.
- Must follow all HIPAA and OSHA requirements.
- Must be able to operate or have ability to be trained to operate office equipment.
- Must be supportive of our mission in order to provide healthcare to the underinsured and uninsured.
- Education: High school or equivalent
- Customer Service: 1 year
LICENSED PRACTICAL NURSE
Looking for a friendly, professional nurse that will provide optimal care to each patient and further the mission of the Primary Care & Hope Clinic.

Essential Functions
- Provide prompt, efficient and accurate patient care.
- Prepare exam room for use and sterilizes equipment and supplies.
- Conduct daily health center room clean-up
- Escort patients to exam room determining chief complaint and pertinent history.
- Provide patient education as warranted communicating laboratory and diagnostic testing results by phone and mail.
- Assist provider with procedures and patient care.
- Call prescriptions into pharmacy.
- Take and record vital signs, record various results (height, weight, vision, hearing, B/P urinalysis, glucometer check, pregnancy test, etc) with re-assessment of abnormalities.
- Perform EKG, venipuncture, suture removal, splinting, breathing treatment, immunizations, audiometry test, IV administration, catheterization, wound care, dressing changes (i.e., nephrostomy and PICC line), etc.
- Utilize eclinical software for patient records.
- Triage calls for provider to include determining emergencies, identifying symptoms, using discretion for work in appointments and refilling routine prescriptions after reviewing medical chart.
- Contact patients, as necessary, on next day’s schedule for pre-visit planning.
- Perform responsibilities in line with Patient Centered Medical Home, including, but not limited to, pre-visit planning and “huddles” with Care Team.
- Perform laboratory tests of minimal to moderate complexity, including completion of lab requisitions per computer and paper.
- Schedules external imaging for patients.
- Complete Prior Authorizations (PA’s) for provider as requested.
- Assure all lab specimens have been picked up at day end.
- Ensure the infection control standards and OSHA standards are followed and hazardous waste is properly disposed.
- Improve the patient experience and health care processes and help assure the provision of high quality health care, as well as an ongoing process of performance improvement.
- Complete all required paperwork.
- Plan and organize work load to maintain efficient patient operations in the Health Center.
- Maintain a strong patient focus while working collaboratively within the teams multiple demands and patient needs.
- Keep work area and patient rooms clean and free of clutter.
- Treat all patient information confidentially and in accordance with established guidelines.
Requirements
- Must have and maintain active Tennessee LPN Licensure.
- Must maintain a CPR Certification.
- Must be able to lift 30 pounds/ walk and/or stand for up to ten hour shifts-bend or stoop throughout shift
- Must follow all HIPAA and OSHA requirements.
