We are accepting new patients who have BlueCross BlueShield or TennCare.

We are accepting new uninsured patients who:

  1. Do not have any form of health insurance coverage
  2. Reside in Rutherford County or Cannon County
  3. Complete intake process with Primary Care & Hope Clinic's social worker

For intake, these items are required:

Proof of Identity

Driver's license,
passport,
or state-issued identification card

Proof of residence in Rutherford County or Cannon County

You can bring a utility bill.

Proof of Household Income
(Required for all household members)

2006-2007 tax returns
3 recent check stubs
Official Documentation for Food Stamps (EBT), Families First (TANF), Social Security/Disability, Pension/Retirement, Unemployment, Worker's Compensation, Interest/Dividend Income, Child Support, Alimony, Rental Income, or any other source of income
