The Primary Care and Hope Clinic is a not-for-profit healthcare clinic providing quality, affordable family health services to medically uninsured and underserved families in Rutherford and Cannon Counties. The clinic operates on a sliding fee scale based on income and family size.
ELIGIBILITY FOR PATIENT INTAKE
- If not working, must be receiving disability, social security, unemployment, child support or families first. (If spouse is employed will base on their income. If someone is financially supporting patient, then the friend or family member’s income will be required).
- May be a full time student and not working.
- If receiving Behavioral Health TennCare can be eligible.
- If a person is with Lazarus House, Room in The Inn , Esther Project or Salvation Army they will be eligible.
ITEMS TO BRING TO INTAKE APPOINTMENT
- Photo ID
- Proof of Residence in Rutherford or Cannon County (ex:utility bill)
- Proof of income in 3 current check stubs, unemployment stub, families first letter, or letter from any organization above.
- Social security income can be provided by letter, bank statement deposit or copy of check.
- Copy of most recent tax return.
- $10 Intake fee per person applying for Hope program.
All items are required for intake appointment or appointment will be rescheduled
