Our Services

The Primary Care & Hope Clinic provides healthcare for all ages. Our medical services include adult and pediatric health, immunizations, chronic illness management such as diabetes, hypertension and high cholesterol, and healthcare for minor injuries.

We use an integrated behavioral health model that teams behavioral health providers with your medical provider. Private, in-house behavioral health therapy for adults, adolescents, and children are with our licensed counselors or our Mental Health Psychiatric Nurse Practitioners.

Our on-site pharmacy serves patients with TennCare, Medicare Part D, and most private insurance plans. We also offer low-cost medications through our 340B discount drug program. The Primary Care & Hope Clinic offers nutrition and diabetes management and education services. This includes Diabetes Self-Management Education, medical nutrition therapy, and diabetic and weight loss support services.

Pediatrics

We are committed to providing affordable, accessible healthcare for children. Our services include Well-child check-ups, immunizations, colds, flu, and sore throats, and treatment of minor injuries.

X-ray & Labs

The Primary Care & Hope Clinic has on-site X-ray and lab that provides testing for blood work, urinalysis, Hemoglobin, STD screening and PAP smears.

Insurance Assistance

The Primary Care & Hope Clinic has Certified Application Counselors on staff to assist with enrolling in affordable health insurance coverage or TennCare through the Health Insurance Marketplace.

Extended Hours
