Open Positions:
Family Medical Doctor
General Qualifications:
- Doctoral Degree in Family or Internal Medicine and be Board Certified
- Completed Residency Program
Schedule: Full Time
Responsibilities:
- Deliver and direct patient care to patients within scope of practice
- Develop, review and implement clinical practice guidelines and protocols
- Act as collaborating physician for Provider staff
Licensed Clinical Social Worker (LCSW)
General Qualifications:
- Master of Social Work Degree with LCSW and current State of Tennessee Licensure
Schedule: Full Time
Responsibilities:
- Work closely with the other providers to deliver an integrated model of patient care
- Counsel and educate patients on personal issues, such as disease management, mental illness, poverty, abuse and addiction
- Conduct individual, family and group therapy, outreach and crisis intervention
- Implement treatment plans in a strengths based model
