Sliding Fee Discount

The Primary Care and Hope Clinic is a non-profit health center providing quality, affordable family health services to medically uninsured and underserved families in Middle Tennessee. The health center accepts all patients regardless of their ability to pay.  We offer a sliding-fee scale discount to all eligible patients, based on annual household size and income.

To qualify for the sliding fee discount, patients must complete the social services intake process to establish their sliding scale fee.  Patients must re-qualify for their sliding fee discount each year.

Patients must complete an intake application and provide evidence of household income that does not exceed double the annual Federal Poverty Guideline.

 

ITEMS TO BRING TO INTAKE APPOINTMENT
  • $10 intake fee per person applying for the uninsured program
  • Photo ID: Driver’s License or other state issued ID
  • Proof of residence
  • Proof of income (can be provided by letter, bank statement or copy of check):
    • 3 current check stubs
    • Copy of most recent tax return
    • Unemployment stub
    • Pension and/or Retirement
    • Social Security
    • Disability
    • Child Support
    • SSI

All items are required for intake appointment or appointment will be rescheduled.
